Supported Living

Practical, person-centred support for adults aged 18+ who want to live more independently, in their own home or the home they rent.

Support that fits your life

We work alongside the people we support to build the skills, routines and confidence to live the way they choose. Support is planned with you and changes as your needs and goals change.

  • Building daily routines
  • Managing a home and tenancy
  • Budgeting and paying bills
  • Cooking, shopping and everyday living skills
  • Getting to appointments
  • Connecting with the local community
  • Building confidence and independence
  • Social inclusion and staying in touch with people who matter

What is supported living?

Supported living helps adults, including people with complex needs, keep their independence while getting the practical and emotional support they need to live a full life in their own home.

People in supported living usually have their own tenancy, often in a home provided by a housing provider, and stay in control of their bills, household tasks and the other responsibilities that come with running a home. Support is provided separately, so it can be built around each person's wishes and goals and help them live as independently as possible.

Your home stays your home

We support people wherever they live, whether that's a home rented from a housing association, a private landlord or another provider, or a home of their own. Because we support you rather than house you, your home stays yours if your support arrangements ever change.

Who we support

We support adults aged 18 and over across Bristol, Somerset, Gloucestershire and South West England who would like support to live more independently.

Referrals are welcome from individuals, families, social workers, housing providers and support organisations.

How we create support plans

We only write a support plan once we've got to know the person and understand their history, their current situation and their goals for the future. We talk in depth with the person, their family and any professionals involved, so we have the full picture and have heard what the person wants before support begins.

Once a support plan is in place, we review progress regularly, recognise every achievement and encourage the person to set new goals.

How our supported living process works

  1. Step 1

    Get your funding in place

Supported living usually needs funding to be agreed first. Contact your local council or GP, who can put you in touch with a social worker to help get your support agreed.

If you find this process hard to navigate, call us on 07532 659253 and we'll do our best to help.

  2. Step 2

    Get in touch with us

Once your local council has agreed your support, contact Lantern Homes. A member of our team will then get in touch with you.

  3. Step 3

    We get to know you

We'll arrange to meet you in person to get to know you better. We'll talk about your current situation, the support you need and your short, medium and long-term goals.

  4. Step 4

    We plan your support together

We'll plan your support around you, with your family and any professionals involved if you'd like them to be, and answer any questions you have.

  5. Step 5

We send a proposal to your council

We'll prepare a support and funding proposal and send it to your local council for approval, keeping you updated throughout.

  6. Step 6

    Your support begins

Once funding is approved, we'll agree a start date and be there to help you through the change, so it goes as smoothly as possible.
